Objectives

NARRATIVE DESCRIPTION

Following this course, the participant should be able to appraise the etiologies and recognize the clinical presentations of the commonly seen disorders presented; indicate diagnostic tests available and the suggested use for making a diagnosis; implement available treatment options and appropriate long-term care when indicated.  This activity is expected to result in improved competence in making an appropriate diagnosis and providing effective treatment and referral or follow-up care with the overall goal of improving patient outcomes.

The emphasis will be on aligning physician behavior with current guidelines and evidence-based medicine, as indicated within each topic’s specific objectives.  Since Internal Medicine takes a lead role in medicine in general, this course was designed for all practitioners at the level of a practicing physician to provide a review and update in their application of medical knowledge and practice strategies to diagnose, treat and improve patient care. 

SPECIFIC TOPIC OBJECTIVES

Day 1

Prevention of Type 2 Diabetes/Treatment of Obesity. (Pendergrass)Upon completion of this session, the participant should be able to: GL, COMP

  1. Recognize patients with pre-diabetes.
  2. Employ current Guidelines and recommendations regarding diabetes prevention.
  3. Describe properties of currently available medications for obesity, their adverse effect profile, and how to incorporate them as part of an effective obesity treatment program.

Treatment of Hyperglycemia in Type 2 Diabetes (Non-Insulin) (Pendergrass)Upon completion of this session, the participant should be able to: EBM, GL, COMP

  1. Understand the glucose targets in T2DM.
  2. Employ the current ADA Guidelines for pharmacological therapy.
  3. Develop individualized therapeutic regimens to optimize glycemic control.

Treatment of Hyperglycemia in Type 2 Diabetes (Insulin) (Pendergrass)Upon completion of this session, the participant should be able to: EBM, GL, COMP

  1. Recognize indications for the initiation of insulin replacement therapy for people with T2DM.
  2. Describe clinically relevant properties of the available insulins and insulin combinations.
  3. Individualize and advance insulin therapy to optimize glycemic control.

Diagnosis of Primary Headache Syndromes.   (Smetana)Upon completion of this session, the participant should be able to: COMP

  1. Distinguish among the historical features of migraine, tension-type, and cluster headaches.
  2. Recognize which historical features best discriminate between migraine and tension-type headaches.
  3. Apply the triggers for migraine to lifestyle modification.

Treatment of Primary Headache Syndromes. (Smetana)Upon completion of this session, the participant should be able to: EBM, GL, COMP

  1. Appreciate the role of abortive therapy for migraine, tension-type, and cluster headaches.
  2. Recognize the range of options for pharmacologic acute and preventive therapy, as recommended by the AHS 2015 Evidence-Based recommendations for migraine, as well as options for tension-type, and cluster headaches.
  3. Develop non-pharmacologic treatment strategies for migraine as per the most recent Cochrane Systematic Review.

Day 2

Hypertension: Recent Trials and Rational Therapy.  (Smetana)Upon completion of this session, the participant should be able to: GL, COMP

  1. Appreciate that all antihypertensive medications do not equally reduce cardiovascular risk.
  2. Apply the current JNC guidelines to clinical care of hypertensive patients.
  3. Recognize compelling indications for selection of initial antihypertensive therapy.
  4. Identify which drugs most effectively reduce the risk of cardiovascular disease.

Preoperative Cardiac Evaluation.   (Smetana)Upon completion of this session, the participant should be able to: GL, COMP

  1. Develop risk stratification for perioperative cardiac risk.
  2. Demonstrate knowledge of preoperative cardiac risk guidelines and the revised cardiac risk index.
  3. Properly use beta-blockers and statins for patients at risk for perioperative cardiac complications.

Common Endocrine Disorders. (Pendergrass)Upon completion of this session, the participant should be able to: EBM, GL, COMP

  1. Recognize, evaluate and treat common endocrine abnormalities and employ Endo Society Guidelines to include:
    1. Hypoglycemia,
    2. Hypercalcemia,
    3. Low testosterone,
    4. Adrenal masses.

Thyroid Disease. (Pendergrass)Upon completion of this session, the participant should be able to: GL, EBM. COMP

  1. Individualize evaluation and management options for  hypothyroidism.
  2. Review evaluation and treatment options for hyperthyroidism.
  3. Review current guidelines describing the evaluation and management of thyroid nodules.  

Common Diagnostic Dilemmas in Primary Care.  (Smetana)Upon completion of this session, the participant should be able to:

  1. Describe the diagnostic pearls for “Can’t Miss Diagnoses”
  2. Avoid “near misses” for these diagnoses
  3. Identify which features of the history and physical examination are most useful to make correct diagnoses
  4. Describe the clinical prediction rules to establish key diagnoses

Day 3

Diabetes Technology.  (Pendergrass)Upon completion of this session, the participant should be able to: GL, COMP

  1. Discuss new trends and hot topics in diabetes technology.
  2. Recognize and identify clinical scenarios where the use of newer technology, such as insulin pumps and/or continuous glucose monitors, may be indicated to support insulin replacement therapy
  3. Discuss how new technologies, such as insulin pumps and   continuous glucose monitors, are incorporated into routine practice.
  4. Describe potential risks and benefits of available nonprescription diabetes management technologies

Transgender Care. (Pendergrass)Upon completion of this session, the participant should be able to: GL, COMP

  1. Identify the differences between sex, gender identity, sexual preference and gender expression and be able to use the appropriate terminology.
  2. Describe how to safely and effectively utilize masculinizing and feminizing hormonal treatments for appropriate patients.
  3. Assess the components of, expected changes, time line and outcomes of cross hormone therapy.

New Drugs for the Primary Care Physician: What You Need to Know.  (Smetana)Upon completion of this session, the participant should be able to: COMP

  1. Survey important advances in pharmacotherapy that pertain to primary care practice.
  2. Discuss the efficacy and list the side effects for each drug.
  3. Differentiate key new drugs with previously available medications for similar indications.

Rational Management of Depression in Primary Care. (Smetana)Upon completion of this session, the participant should be able to:  GL, COMP

  1. Employ an appropriate initial approach to use of the updated USPSTF Guidelines for screening of the patient with depression.
  2. Determine the proper role of second generation antidepressants as per the ACP Guideline.
  3. Assess the effectiveness of augmentation and switching strategies.
  4. Specify the most common side effects, as per the 2016 ACP Guidelines, of commonly used antidepressants.
